Anal fissure is a tear or cut in the wall or opening of the anus, which causes severe pain during bowel movement accompanied by minimal bleeding.
Common causes of fissure are frequent constipation, diarrhea, childbirth, anal sex, and any other practices that may cause trauma to the anal region. 1. Kshara karma
It is a nonsurgical procedure wherein the sentinel tag (pile mass that is present in severe anal fissures) is ligated using specialized medicated alkaline thread. In a sense, it is cauterization with the use of chemicals (naturally prepared). The thread immersed in ayurvedic healing oils is placed tightly around the affected area or sentinel tag with the help of a special slit protoscope, which causes the tissue to be strangulated and then fall off in 3-5 days. After the sentinel tag is removed, the underlying fissure closes up in a week or two.
2. Avagada sweda
It is a type of sudation (stimulation of sweat glands) therapy, where the patient is immersed or made to lie in a tub containing Dravadravya (medicated water) to ease the body of pain. Technically, it is a sitz bath with Indian medicated decoctions.
A tub is filled with 10-15 liters of medicated water with a temperature of 38-42 degrees Celsius. The patient is seated comfortable in the tub with the Dravadravya reaching up to the umbilicus. The Dravadravya should always be kept at the desired warmth, thus requiring frequent replacement. Therapy is stopped when the patient achieves good perspiration. Afterwards, the body is wiped with a towel and the patient is allowed to rest for 30 minutes to 1 hour and then instructed to have a bath in lukewarm water.
Complications of therapy include fainting due to low heat threshold of patient or increased body temperature. This can be reversed by providing water supplement and/or sprinkling cold water on body and face. Some patient sustain burns due to excessive heat of Dravadravya or heat intolerance. Microvascular and peripheral circulation conditions should be ruled out before subjecting patient to any heat therapy.
These procedures should be supplemented by diet modifications in order to achieve better results. Digestion is enhanced by increasing fiber and water intake (preferably warm water). Avoidance of spicy hot and citrus foods will limit gastric distress. Yoga and other meditative techniques promote relaxation which helps with pain and stimulating bowel movement.
